Children with congenital conditions like cleft lip and palate frequently need surgical procedures in addition to orthodontic treatment for effective rehabilitation. Recently, new surgical techniques have been incorporated into orthodontic management, including the Surgery First approach, which significantly shortens treatment time; implant placement to provide additional anchorage; and distraction osteogenesis to advance the maxilla and mandible. It is estimated that about 5% of the Indian population has dentofacial deformities that cannot be effectively treated with orthodontics alone and require orthognathic surgery as a definitive treatment. Orthognathic surgery can be employed to correct: - Major jaw misalignments (including anteroposterior, vertical, or transverse issues). - Facial skeletal imbalances linked to airway defects and soft tissue inconsistencies. - Facial skeletal issues related to identified temporomandibular joint disorders.
For whom surgery is indicated? Jaw surgery may be carried out for patients with significant discrepancies between the upper and lower jaws. Upper jaw (maxillary osteotomy) A maxillary osteotomy may be performed to correct these issues: - Significantly recessed or protruded upper jaw - Crossbite - Excessive or insufficient tooth visibility - Open bite From inside the mouth, the surgeon makes an incision in the bone above your teeth, allowing the entire upper jaw and upper teeth to be repositioned as a single unit. The jaw and upper teeth are then shifted forward until they align correctly with the lower teeth.
